An assessment, a visit by a nurse and/or health advocate to identify problems, provide concrete options, and offer objective professional solutions. Recommendations and referrals may be made. Often this is all that is needed; the family/friend is able to "take it from there". There is no further obligation. The fee for this service ranges between $500 and $1,500, depending on variables, such as: travel, time completing the assessment and report, and testimony (if necessary) as an expert witness.

Once the assessment has been completed and the family/friend has determined that further help is needed a care plan is developed and completed. The care manager implements and oversees all aspects of the care plan and reports to the family/friend as previously determined. Each care plan is designed to meet the specific needs of the client.
